Case Management
CCDS is fortunate to have on-site case managers who take pro-active roles in assisting individuals with developmental disabilities enrolled in our various programs.  These measures include identifying individual needs, establishing support systems, developing service plans, linking to appropriate services and measuring outcomes. CCDS case managers work closely with the individual, family members, support agencies, and community resources to help each individual reach his or her potential.
 
While the CCDS case managers must maintain comprehensive records (narratives, tracking sheets, etc.) they also have the unique opportunity to really get to know the people they serve.  In helping individuals become more independent, they may assist with Social Security issues or may even work with those who seek residential placement - or even their own homes!  Case managers also participate in advocacy events (People First, Disability Day at the Capitol) and attend training and forums held by Georgia's Department of Human Resources (DHR) to remain current on all issues affecting the lives of the people they assist.

Beginning in the Summer of 2008, all case managers began certification through the College of Direct Support.  The 17-course program includes topics like Individual Rights and Choice, Introduction to Medication Support and Cultural Competency.  All case managers will be certified by Spring 2009.
