Coastal Center for Developmental Services has trained, on-site Case Managers who
take pro-active roles in identifying consumer needs, establishing support systems,
developing service plans, linking to appropriate services and measuring outcomes.
Case Management services are provided to all CCDS consumers in Day Habilitation,
Organizational Employment Services,
and Community Employment Services.
Consumers and their families receives support through advocacy, coordination,
and linkage with community resources and support services.
Consumers are assisted in identifying goals and developing an Individual Service Plan (ISP)
designed to achieve maximal self-sufficient and community inclusion opportunities.
Case managers work with consumers and their families in describing their needs
and defining the means to meet these needs.
Case managers maintain chronological records that include narratives, tracking sheets,
and/or charts. Maintained by instructors and staff, these activity notes help document
consumer progress or regress relative to desired ISP outcomes.
